The evil alien Waroga appears again along with an ominous declaration of war. The Defense Force and Team EYES engage the enemy. But the flow of battle turns to Waroga’s advantage and Musashi is heavily wounded without being able to turn into Cosmos.

- The initial script for this episode was actually written by Hideki Oka. Before writing, Masaki Harada requested changes to the plot, resulting in a script titled "Enemy Within the Heart," which depicted Waroga as a being who manipulates human life, similar to the portrayal in episodes 13 and 14:[1]
- Dr. Kawaya and Shinobu's relationship reaches the engagement stage. However, Miyako, a woman whom Kawaya had previously been unable to cure, suddenly reappears before him. Through Miyako's explanation, Kawaya learns of Waroga's plot: the children suffering from the same rare disease as Miyako have their immune systems compromised. Waroga intends to use their bodies as hosts for resurrection. Due to humanity's fear of extraterrestrials, public opinion and JADF view these children as threats. To protect the children, EYES remains stationed at the sanatorium.
- In reality, Miyako is the host whose body has been taken over by Valorga. Upon discovering the truth, Shinobu shoots at Miyako, and Valorga appears and is defeated by Ultraman Cosmos. Ultimately, Kawaya leaves for overseas to research a vaccine that could save the children. Shinobu, who watches Kawaya leave, still wears the engagement ring he gave her.
- Producer Hiroyasu Shibuya later deemed the story too heavy for the audience, leading to a last-minute rewrite by Masakazu Migita. Some elements of the original plot were incorporated into episode 47, "Sorceress of the Sky."
